45 min class = $52.00/month

Parent/Guardian Signature: I willingly grant permission for photographs/video of my child to be published for Dance House advertising via print media and the world wide web. I agree that I am enrolling for a full season. I agree responsibility for the full season's tuition, which is broken down into monthly payments, that are due the 1st of each month. I agree to pay a $10.00 monthly late fee (for each month of late tuition), for tuition payments received by Dance House after the 8th day of the month they are due. Payments may be made online or by cash/check. Checks can be mailed to 208 Pine St. Highland, IL 62249. A $35 returned check fee will be added to your account for any checks returned unpaid. Students will not be allowed to participate in classes if tuition payments are more than 60 days past due. I acknowledge that there are NO REFUNDS.
