Thank you for providing the contact information for the site(s) using recycled water for irrigation. Please note that you only need to complete this survey once for each site that uses recycled water. For instance, if your site has ten recycled water irrigation meters/accounts with SMWD, simply fill this form out once for the entire site. However, if you manage completely separate sites (ex. Valmont HOA as well as Shoreline HOA) then you need to fill this form out twice for each separate site that uses recycled water.
