*Please complete this form & make your deposit to get on our Screen Printing List. *NO Artwork will begin without deposit! *Our current turn-around for a proof is roughly 4-6 weeks, this may vary, especially during our busy season! *Once a proof has been approved, you will fall into the print order as orders are received and deposits are made. *Shirt Art is for SG ONLY and will not be shared or sent for other sources to print.
Deposit Information

A Deposit of $200* for Team Tees/Logo Tees or $500* for Race/Custom Tees and is due upon submitting this form - this goes towards your final invoice or artwork fees.
Artwork Fees

Team/Logo Tee Artwork is $200. Race Tee/Custom Artwork is $500. This fee is to cover the time put in to making your design if we have to draw new artwork. There is NO artwork charge if Sybesma does your car & no charge on orders over 200 pieces. Artwork includes 1 car per shirt on a race tee. A second car option can be added for $200 (SEE PRICING SHEETS BELOW)
