Instrument Usage Contract

The Department of Fine and Performing Arts (DFPA) maintains a limited inventory of musical instruments that are rented to students on a first-come, first-served basis, but demand often outpaces supply. Highest priority is given to students using instruments during ensemble class periods, then to students using instruments for a DFPA-organized lesson, class, or rehearsal. Students interested in using a DFPA-owned wind or string instrument should complete and submit this form. After submission, the Operations and Concert Manager will respond to confirm the availability of the instrument requested, and provide instructions for payment and pick-up.

I understand that abuse of the policy may result in the supsension or loss of instrument rental privileges. I agree that I am responsible for the care and maintenance of this instrument and case while they are in my possession both inside and outside of academic activities. I understand that this instrument is to be used for official Loyola University Chicago Department of Fine and Performing Arts activities only, and agree that I will not allow any other party to possess or play the instrument without consent from the Operations and Concert Manager. I understand that I may be charged for missing accessories, damage from negligence, or failure to return the instrument when requested.
