Thank you for submitting a check request for Group childcare. Please keep the following guidelines in mind when requesting payment.
- Requests should be submitted after the group has met.
- Submit your request no later than the Monday after the group meets. Checks will be printed weekly and mailed directly to the childcare worker.*
*For tax reporting purposes, payments will be made directly to the childcare worker rather than reimbursing the Life Group leader for childcare costs.

All childcare workers should be informed of the following: If the total amount of payments given in one calendar year exceed $600, these payments will be considered taxable. Focus Church will request completion of a W-9 Form by the worker, and a Form 1099-MISC will be issued at the end of the tax year.
