PLEASE NOTE: Documentation and Insurance Requirements:
- Copies of Business Registration (corporate paperwork) & Tax EIN number
- Copy of the Food Handler’s Certificate
- Proof of Insurance
- General Liability, including Workman’s Comp if there are employees) with coverage of at least $2,000,000 per occurrence.
- Product Liability: Should User be manufacturing product for distribution, they will be required to show product liability & Completed Operations Coverage insurance in the amount of $2,000,000 per occurrence.

- General Liability, including Workman’s Comp if there are employees) with
Auto Insurance:Appropriate coverage for non-owned and hired auto coverage.
- Application Fee of $50 is required at time of application. This fee is applied to your first month of facility membership.
***Important Note:All members need to obtain insurance as outlined above. This insurance should be in force at the time of the membership agreement signing and for the duration of User’s affiliation with the facility. You will be required to provide renewal certificates at time of insurance policy renewal.
Users must add the kitchen as an additional insured. Here is the exact wording that must appear on the COI:
