Fill out this form to request data from the Institutional Research (IR) Office. This form helps the IR Office gain a better understanding of the data needs of the institution. Should you have any questions or have trouble filling out the form, please contact the IR Office at institutionalresearchteam@stmartin.edu or 360-412-6145.
It is important that you read and understand the terms of the IR Data Policy & Responsibilities prior to submitting this form. Data requests that deviate from or violate the policy will not be fulfilled until they are approved by the Provost or President. The IR Office will do its best to ensure you receive the requested data by the selected urgency level. Should the IR Office not be able to meet your deadline, you will be notified.
