  • Intended use:  This form shall be used for all activities occurring in the United States (mini camps, Annual Meeting & Conference/CISV In Motion, National Youth Meeting, National Leadership Training, etc.), by youth under the age of 18.  
    This form is to be completed by the parent or legal guardian of the participant.  Completion and signing of this form is a condition of participation in the CISV activity named below.

    Event Address: Camp St. John, 235 Marywood Drive St. Johns, FL 32259 (off SR13, 3 miles south of Racetrack Rd.)

  • Phone Policy

    CISV activities are cell phone free. Only chaperones and staff (JB Board members) are permitted to have their phones during their minicamp. Participants are encouraged to leave their cell phones at home and not to bring them to minicamp. CISV will store any cell phones that are brought to minicamp at a campsite location in the chaperone's discretion and will return them at the end of camp. CISV is not liable for any loss or damage to cell phones.

    I give my child permission to register on https://www.cisv.org/mycisv/myCISV is designed to assist CISV with administration of the CISV activity named above and to help CISV participants stay in touch with each other after the activity.

  • I understand the nature of the CISV Activity noted above and I consider my child to be capable of taking part in it. I agree not to make a claim or file a lawsuit against CISV (1) if my child is injured while traveling to / from and/or participating in the above Activity, unless there has been gross negligence on the part of CISV and (2) for any lost or damaged property. My child and I understand that CISV participants are expected to conduct themselves in accordance with local laws and CISV rules, including the Code of Conduct. If my child engages in inappropriate behavior he / she may be sent home before the end of the Activity at CISV’s discretion. I will cover the costs of this trip. I also agree to pay for any damage or injury caused by my child. Parents should sign below to indicate your agreement with the statements above.
