Ascent Houma Facilitator
Annual Salary: Negotiable based upon experience
Ascent Health, Inc., Ascent, is a non-profit organization offering intensive, individualized care planning and management processes for children and adolescents with complex mental health and/or other needs. These services are offered to young people who have involvement in multiple child-serving agencies (i.e. Office of Juvenile Justice, Department of Child and Family Services, Office of Behavioral Health, Department of Education) and whose families would thus benefit from coordination of efforts across these systems.
Responsibilities:

Work comfortably with youth and families experiencing complex challenges and crises.

Serve as the primary point of contact for youth and families involved in multiple systems.

Develop a Child and Family Team which includes formal and informal supports and services.

Create an individualized Plan of Care in partnership with the family and team that adheres to National Wraparound Initiative standards.

Assist youth and families with accessing strength-based mental health services and other supports and resources needed to achieve the Plan of Care.

Monitor and evaluate the Plan of Care to ascertain whether certain services are being provided appropriately and are sufficiently meeting the needs of families.

Develop positive relationships with community-based providers in order to ensure access to quality services for youth and families.

Comply with all required documentation and data collection in support of the program’s implementation and evaluation strategy.
Requirements:
- Bachelor’s level degree (Master’s preferred) in human services related field.
- Minimum of two years of full-time experience working in relevant family, children/youth or community service capability.
- Access to reliable transportation. If driving an automobile while on company business, must possess a valid driver's license and auto insurance.
