This questionnaire is designed to help Luvmydoodle and you determine if our Bernedoodles,Sheepadoodles and now Cavapoos are the right fit for you and your family. All adopting parties are required to complete this form prior to adoption. Luvmydoodle reserves the right to deny any adoption based on answers to this questionnaire and phone interviews prior to adoptions!

We will be more than happy to meet you at Norfolk Internation Airport to pick up your puppies and we can recommend flight nannys too.
