Luvmydoodle Puppy Application Questionnaire

This questionnaire is designed to help both you and Luvmydoodle determine if one of our Bernedoodles, Sheepadoodles, or Cavapoos is the perfect fit for your home and lifestyle.

Because our puppies are raised in our home and are deeply loved from the very beginning, we are intentional about placing them with families who are equally committed to providing a lifetime of care, love, and stability.

All prospective families are required to complete this questionnaire prior to adoption. Luvmydoodle reserves the right to approve or deny any application based on questionnaire responses and a follow-up phone conversation, as we strive to ensure the very best match for both our puppies and their future families.

We are happy to assist with travel arrangements. We can meet families at Norfolk International Airport for puppy pick-up and are also happy to recommend trusted flight nannies if needed.

Please note: All Luvmydoodle puppies are placed as family companions (pet homes only). Breeding rights are not available.
