DXC Technology are proud to offer all DXC Technology Employees access to a FREE Onsite Health Check

We all know the importance of looking after ourselves yet with our busy lifestyle and various commitments it's sometimes challenging to find the time or place to have a Health Check.

This year's Onsite Health Check is bought to you by the DXC Health Benefits Plan and partners, Anista Employee Benefits and Bupa Wellness.

What is included in an Onsite Health Check?

The Bupa Wellness nurse will look at the below parameters.

Key Biometrics:

- Blood Pressure

- Total Cholesterol

- HDL Cholesterol

- Cholesterol ratio

- Blood Glucose

- Waist circumference

- Height/waist ratio

- Type 2 diabetes

- Cardio Vascular Disease risk

Lifestyle factors:

- Smoking

- Physical activity

- Alcohol

Discussion

- Goal setting

Please note, an onsite Health Check does not replace your own Doctors' Check-up, but rather gives you the opportunity, whilst at work, to talk to a nurse and possibly identify any issues that would need further investigation by your own doctor.*

A reasonable alternative to an Onsite Health Check at work is a Pharmacy Health Check.

AMCAL and Priceline Pharmacies provide such Health Checks, each focusing on men and women respectively. For more information on each, we have provided links below for your convenience.

Special note: Pharmacy Health Checks do not and should not replace your annual GP's Health Check. A Pharmacist Health Check is simply an easy and convenient service that may provide a good insight into your general health. The Pharmacist will, in fact, encourage you to seek further advice from your Doctor should they identify any issues or have any concerns about your health.

Your reimbursement: The AMCAL Health Check is free of charge. The Priceline Health Check is $9.95 after you have joined their Sisters Club. This charge will be reimbursed to you by Anista Employee Benefits as part of the DXC Health Benefits Plan.

Once you have had your Health Check, simply complete the Pharmacy Health Check Claim Form (option 3 above). This reimbursement is only available for the first 100 DXC employees claimants.

Priceline Pharmacy - Women's Health Check includes:
- Lifestyle discussions
  Discuss your diet, exercise and smoking habits and get some handy tips.
- Cholesterol test
  A blood test to determine total cholesterol. Please note: you do not need to fast for this test.
- Breast check instruction
  Learn what a lump feels like with a model latex breast.
- Blood pressure reading
  A simple test done with a cuff around the arm.
- Anaemia screening
  A pinprick blood test to establish haemoglobin levels. This can indicate if there’s a risk of anaemia.
- Weight evaluation
  Weight and height are evaluated to provide a BMI (Body Mass Index) number.

ONLINE REIMBURSEMENT FORM

Available to anyone who is unable to attend an Onsite Health Check
For employees who received their Health Check at a Priceline Chemist, please complete the below reimbursement/claim form.

Please note: The $9.95 reimbursement is only available to the first 100 claimants. One claim per DXC employee. Claims must be lodged before 1st December 2018.

$9.95 is the cost after you have joined the Sister Club. Joining the Priceline Sister Club is free of charge.
