Since VoIP service allows use of telephones located anywhere in the USA the location of each User/Extension must be accurate. List every location where Users are to be located.

IMPORTANT: 9-1-1 EMERGENCY CALLING / EMERGENCY RESPONSE REQUIREMENTS

According to the Ray Baum's Act, if your place of business meets any of the following criteria you will need to add a Dispatchable Location Identifer for each location:

Multi-Story
Multiple Buildings
Remote Location

This will aid emergency responders to quickly locate the source of the 911 call. Take care to list EACH location accurately.

I understand that incomplete or incorrect addresses in emergency calling services are subect to fines of $275 or greater by the US Government. I understand that these fees will be passed on to me.

This is the phone number where calls can be diverted if service cannot be delivered to you at your location. We are happy to advise you that we maintain a five-9's server operation (99.999% up time) but if something does happen the Failsafe Number is the destination where calls will be sent.

By submitting this information I understand that incomplete or incorrect address in emergency calling services are subject to fines of $275 or greater by the US Government. I understand that these fees will be passed to me. XBLUE must disconnect service when no emergency address information has been received within 10-days of service origination. I acknowledge it is my responsibility to update address information in the event it changes (move/postal changes).
