I herby agree that the following; To register for this event the youth/teen must be 19 years old or under and have a autism diagnosis. The named youth or teen must also have a current intake form on file after 8/1/2018

Event: OaSN Youth/Teen Outing (Up to age 19)

Where: Timberline Farms

Address: 3200 Southeast 115th Street
Belleview, Florida 34420

When: Saturday, October 13, 2018

Arrive: 1:45pm (meet at ticket booth) to enter as a group at 2:00pm
(If you do not enter with the group, you will be charged full admission price)

End Time: Closes at 8:00pm

Cost to OaSN families: $5 for Adults (18 and up) and FREE for kids (up to 17 years old)
Some vendor activites are not included in the admission price
