    • Proposal for a new course

    If you need to reference existing course information, you can search Self-Service to view details without logging in.

    Course Changes

    • Complete a separate course change form for each course you are changing. If you are making several changes to the same course, all of those changes can go on one form. 
    • Examples of Course Changes
      • Course title change (name of the course)
      • Prerequisite/corequisite changes that do not change the course/ program objectives
      • Revising a catalog course description
      • Reactivation of a course without changes to objectives
      • Credit or contact hour changes
      • Change of Student Learning Outcomes

    Program Changes

    • You can list more than one change to the SAME program on ONE program change form. Summarize all changes in the description box in the form.
    • Examples of Program Changes
      • Program title change
      • Adding/Removing a course from a program
      • Changing program elective options
      • Changing graduation requirements
      • Change to the General Education requirements
      • Changes to the program guide that reflect articulation agreement changes
      • Program being deactivated
      • NOTE: Pathways are SEPARATE from Programs.

    New/Additional Program Proposal

    • New program
    • Additional program
      • The actual program may already exist, but you are changing/adding new requirements that require an additional degree designator (AAS.XXX or AA.XXX) to be created in addition to the existing degree.
    • Reactivation of a program 

    Information Form

    • Changes to a Pathway that DO NOT change the program itself
    • Typos or grammar edits in title or course description, program guide, etc.
    • Program guide changes in appearance/formatting updates
    • Changes to DQP (Degree Qualifications Profile) Proficiencies
    • Course Fee Change
    • Informational updates that do not change existing course or program objectives, course name, or course prerequisites or corequisites.
    • Any other general informational items to share with the committee.

    Course Deactivation

    • Use to deactivate any courses that will no longer be scheduled.

    If you are unsure, contact your Curriculum Committee Chair. If you submit and realize an error, this can be corrected without redoing the form.
