Around the world, Quotarians change lives for tens of thousands of women and children and the deaf, hard-of-hearing, and speech-impaired. With the introduction of this matching rewards program by the Board of Directors, your Quota Clubs can do even more.

For your club to receive international recognition for your efforts and a matching grant of $100 - $1,000 USD, please answer a few quick questions in as much detail as you can. These can be completed in as few as 5 minutes. Questions noted with a red asterisk must be answered.

IMPORTANT NOTE: Clubs are limited to one application/award per year, so please be certain that you have met the requirements for the desired level before applying.

 

You may close this form anytime and return to your saved information as long as you use the same computer or device. All answers will be saved EXCEPT any uploaded photos - they will need to be uploaded again. A thank you screen will notify you that your entry has been accepted -- once you hit the 'submit' button.

After clicking "SUBMIT" at the conclusion of the form an email will be sent to the address you provided below from staff@quota.org containing:

1) An "EDIT RESPONSE" button, should you need to edit your answers after submitting them.

2) A PDF of your responses attached, should you want to send it to anyone in your club or region.

BRONZE STAR CLUB - $100 USD matching grant

Base Criteria*:
Community Giving: $1,000 USD or more

Program Giving: $300 USD or more

SILVER STAR CLUB - $200 USD matching grant

Base Criteria*:
Community Giving: $2,000 USD or more

Program Giving: $600 USD or more

GOLD STAR CLUB - $500 USD matching grant

Base Criteria*:
Community Giving: $5,000 USD or more

Program Giving: $1,500 USD or more

SUPER STAR CLUB - $1,000 USD matching grant

Base Criteria*:
Community Giving: $10,000 USD or more

Program Giving: $3,000 USD or more

Clubs who meet ALL requirements listed below will receive a $200 matching grant from Quota International, Inc.
