The online application and support documents can be submitted on this page by completing the application below and uploading all required documents.
If your application is submitted online, an invoice will be emailed to you within three (3) working days. When you receive the invoice, please follow the provided instructions to submit payment. Your application will be processed after payment is received. Once your payment is processed, a building inspector will inspect the business location in 3 days. Please note, for commercial applications, the Fire Marshal inspection and Commercial Location Business Use Verification report is required BEFORE submitting an application.

Commercial Location Business Use Verification Approval**
Fire Marshal Inspection Report**
Notarized SAVE affidavit
Notarized E-VERIFY affidavit
Copy of Drivers License
Copy of any exemption documents
Any applicable state, local and professional licenses
