Welcome to the financial church report packet. This report has four sections:

The forms from this packet will be automatically sent to the Finance Department once you hit “Submit” on page 4. You will also receive a pdf copy via email. Payments can be submitted by Debit Card, Cash App, or P. O. Box.

Appointment License Assessment Form
Please note that the license assessment fee is $5.00 each. Pastors and Superintendents do not have to pay the $5.00 fee for their Pastor Appointment and Superintendent Appointment licenses. The form is set up to enter 35 members. If you will need more than 35 members, send an email to brendon.hamner@houseofgod.org before you begin completing the form.

*Church Assessment amounts are as follows: 1 to 25 members ($150 or more), 26 to 40 members ($300 or more),
41 to 55 members ($450 or more), 56 to 70 members ($600 or more), 71 to 85 members ($750 or more),
86 to 100 members ($900 or more) and 101 members & up ($1,050 or more).
