Trunk or Treat is a safe and fun way for families and children to enjoy Halloween

EVERYONE MUST WEAR A MASK AND PRACTICE SOCIAL DISTANCING.
All cars and trucks are welcome and must complete registration form prior to the event. Spots are limited, register soon! Vehicle owners do NOT need to come in costume (although strongly encouraged), however, ALL vehicles MUST be decorated. First, Second and Third Place awards will be awarded, as well as a “People’s Choice” award.
Trunk or Treaters will spend the evening going from trunk to trunk from their car, collecting candy from vehicle owners who have decorated their vehicles to serve as the backdrop for this family friendly event. “Trunkers” will be asked to decorate their trunk, tailgate or hatchback and provide candy for the “treaters”.
RISK, WAIVER AND IMAGE RELEASE FORM
The undersigned hereby agrees to defend, indemnify, and hold harmless the park district and its officers, employees and agents from and against any and all loss, liability charges and expenses (including attorney fees) and cost which may arise by reason of participation in any program. (The park district does not provide accident, medical, liability, workers’ compensation insurance or any other insurance for program participants). As parent/guardian, I hereby consent to emergency treatment of my minor child as a result of accident or injury. I further agree to pay any and all cost incurred as a result of said treatment. I agree to carefully inspect and satisfy for myself that the facilities provided are reasonably safe for their intended use. Once having conducted the inspection, I agree to expressly assume the risk of participating at the premises. I understand the park district retains the right to use photos taken during activities for publicity purposes.
I acknowledge that my participation in this event, or that of my child, could involve risk of physical injury or damage to property. I expressly assume such risk and release and waive any claims against Blue Island Park District, its agents and employees for any injuries to persons or damage to property.
The Blue Island Park District Recreation Department may use photos, film, videotape, or otherwise reproduce the image and/or voice of any person who participates in any Recreation program, class, or special event and use the same for any purpose without any payment. Your participation in any event or program constitutes your permission to use your likeness.
Waiver and Release of Liability Relating to Corona-virus/COVID-19
COVID-19 is contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.The Blue Island Park District has put in place Operating Policies and Procedures, approved by the Cook County Health Department, as a means to reduce the spread of COVID-19 during the re-opening of park district facilities and programs. I, for myself and on behalf of my child(ren) willingly agree to comply with these policies and procedures. However, I understand that participation in activities and programs with the Blue Island Park District (“Park District”) includes possible exposure to and illness from infectious diseases, including COVID-19, even when these policies and procedures are followed.Therefore, in consideration of being permitted to enter Park District facilities or allowed to participate in Park District programming, I acknowledge the contagious nature of COVID-19 and the risk of injury that may result from exposure or infection to the same. I voluntarily agree to assume these risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at facilities operated by the Park District or participation in programming at the Park District (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Park District, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Park District, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Park District program.I further expressly agree that the foregoing assumption of risk, release and waiver of liability, and indemnity agreement is intended to be as broad and inclusive as is permitted by the laws of the State of Illinois and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.
I have carefully read, I fully understand, and voluntarily sign this assumption of risk, release and waiver of liability, and indemnity agreement and further agree that no oral representations, statements or inducement apart from the foregoing written agreement has been made.
