This is the contract form for Mountaineer Pet Transport
All pets must be at least 8 weeks of age to be transported. The individual requesting or arranging for transport certifies that the pet being transported is at least 8 weeks of age on or before transport pick-up date. The person signing this form certifies the age of the pet to be at least 8 weeks old.
A valid health certificate issued by a doctor of veterinary medicine is required no sooner than 30 days before the scheduled pick up date. The certificate must travel with the pet. Medical records or vaccine records cannot be used as a substitution. All paperwork must be in a folder with the animals name and birthdate on the front and available at pick up. Copies can be emailed if needed but must be done prior to pick up.
A current rabies vaccination certificate must travel with your pet (for pets over 16 weeks of age).*
50% of the total transport fee is due at booking. The remaining 50% is due 24 hours prior to pick up date. Invoices will be sent via Paypal for deposit, you do not have to have a Paypal account. There is be a 4% processing fee for credit card and Paypal payments.
I will require an ample amount of food for your pet in a container or bag that can be closed/sealed securely. No paper or plastic supermarket bags. Please label your food bag with your pet’s name and feeding instructions (amounts and frequency of meals). Your pet will have access to fresh, clean drinking water. I will provide all bedding, water, and crate unless otherwise specified. You may send a pet bed depending on size. I do recommend sending the animals favorite treats, chews and a blanket with familiar smells as this makes travel much easier.
I may not transport any pet that requires a muzzle or other restrictive device. I reserve the right to refuse transport of any pet that displays signs of aggression.*
It is a condition of transport that I am authorized to seek veterinary attention should this be required while animal/s are in my care. Owner agrees to send a signed letter stating I am authorized to seek such services prior to the animal being picked up. Any veterinary charges incurred are the owner’s responsibility. It is the owner’s responsibility to inform myself of any illness, medical conditions or special needs relevant to their animal prior to the animal being picked up.
Scheduling - Pick-up. You will be given a pick up date and a delivery date at booking. I will make every effort to adhere to dates specified by you, but there can be unforeseeable circumstances which affect the dates given for transport which can include weather, road conditions, acts of God, etc. If no person is present at the time of pick up and/or the above described paperwork is not in order at the time of pick-up, the animal will not be transported and 50% of the transport fee will be refunded.
Scheduling - Length of Transport. You understand that there is no guarantee or promise made as to the length of time necessary to complete the transport of your pet. The duration of transport is determined by routing, weather, and traffic conditions. All efforts will be made to keep you informed and to keep your pet as comfortable and happy as possible. You will be provided with estimated pick up and delivery time. I will make every effort to arrive at convenient times, however, scheduling changes and unforeseen delays beyond my control may occur. I will make contact throughout the travel period via phone, text, and facebook messenger.
Scheduling - Delivery. We will make every reasonable attempt to notify the person receiving the pet of our estimated time of arrival. If no one is available to receive the animal and/or we are unable to contact the responsible party, there will be a daily fee of $100 per pet to defray the cost of care and boarding. The party ordering the transport will be responsible for these charges. I recommend that alternate arrangements be made in the event the person responsible for receiving the animal is unable to do so and I be notified of those approved to accept delivery of animal. The receiving party must be available 24 hours a day.
I will make every effort to care for the pet while being transported. Every care will be taken not to expose the animals to any sickness or allowed to be near or in any harm while in my care. Owner, in consideration of the agreement with Amanda Wright to transport the pet, you hereby irrevocably and completely releases her or any helpers from any and all liability, claims, suits, demands, and judgments of any and every kind resulting from or in any way connected with the transportation of the pet and any harm, injury, sickness or death which may befall the pet while being transported. Pet Insurance is recommended and should be purchased by owner prior to pick up. If purchased please be sure to send a copy of policy before the pick up date.
Cat bites, in particular, can be dangerous and require medical attention. Due to the serious nature of the bites I do not place cats in their kennels or chase them upon arrival. It is important that all pets on board trust myself and it is my belief that it is best for the pet parent pickup contact be the one to kennel cats. It is expected the owner or the pick up contact will place the cats in their carriers or kennel.*
If for any reason the transport is canceled by the person who booked the transport within 72 hours of pick up there will no refund of monies paid at booking. If before the 72 hours there will be 75% refund issued of monies received at time of booking. If cancellation has to be made by myself then monies received will be refunded. Refunds will be given within 30 days of cancellation.
Amanda Wright reserves the right to refuse transport of any pet due to misrepresentation at the time of scheduling as to the pets ability to walk or stand of its own accord, or on a leash, and is of a size and weight that one person is able to pick up and carry.*
