I agree to abide by the regulations of the EDAIC Part II examination posted on the ESA website.
I agree with the EDAIC Part II cancellation/refund/postponement policy posted on the ESA website (see bottom of the page).
(1) A certificate of registration as a specialist in anaesthesiology issued by the Medical Council of one of the countries listed as eligible for the Hypnos Grant (and, should this document not be written in one of the languages used for the EDAIC Part I or Part II examinations, a certified translation into English).
(2) A copy of your diploma in anaesthesiology (should your diploma not be written in one of the languages used for the EDAIC Part I or Part II examinations, a certified translation into English is also required).
A letter written in English from your department certifying you are in the last year of your specialist training (the last year of training must start either before or on the day of the Part II registration deadline).
(3) A short CV written in English language (maximum 1 page).
(4) A recent passport-sized picture of yourself.
You are about to submit your application for the Hypnos Grant.
Please note that it will not be possible to modify your application once you have submitted it.
