I (hereinafter referred to as adopter stated in section II), agree to take good care of dog rescued by the DHRA (hereinafter referred to as Doghouse Rescue Academy in section III) for a trial period to assure this dog will fit the family and their lifestyle.
- Foster to Adopt: DHRA and Foster Home agree the dog is being fostered with the intent to be adopted by the Foster Home. At any time during the Foster to Adopt period, DHRA or Foster Home can decide against adoption. At the end of the Foster to Adopt period (14-21 days) a DHRA representative will perform a final evaluation before an adoption contract is issued.
- The dog will be kept as a house pet. The dog will never be tied to any inanimate object while in foster care. At NO TIME will the dog be outside unleashed or loose.
- DHRA does not warrant the temperament or behavior of the dog and is not to be held liable for any acts of the dog while living with the foster home.
- The dog’s interaction with children must always be supervised by an adult listed here.
- The dog shall be confined to a crate or safe area when left alone for the protection of both the dog and the Foster Home’s furniture and belongings. DHRA will not be held liable for damage to home or belongings. This condition will remain in force until it can be established that this dog is not destructive.
- If Foster Homes does not adopt the dog, Foster Home has neither the authority nor the responsibility for permanent placement of the dog. DHRA Rescue Chair may consult with Foster Home for assistance with permanent placement; however, it is the sole responsibility of the Rescue Chair to make such placement. Foster Home agrees to make dog available for viewing by potential adoptive families through coordination of the Rescue Chair.
- Foster Home agrees to return pet to DHRA upon request. Rescue Chair will be provided reasonable notice to return the pet safely to a DHRA representative, in the event Foster Home can no longer continue to foster the dog. Foster Home shell never turn this pet into an animal shelter or any person other than a pre-approved representative from DHRA rescue. Rescue Chair will be notified IMMEDIATELY if the dog is lost/missing or becomes ill.
- Foster Home will pay for, and will be reimbursed by DHRA, for all medical expenses related to the dog, under the direction of the Rescue Chair. These expenses may include, but not be limited to, vaccinations, fecal/heartworm testing, Heartworm/Flea/Tick preventatives, spay/neuter. Medical expenses/procedures other than EMERGENCIES (i.e., bloat, broken bones, shock, internal injuries) for which Foster Home has not received prior approval by Rescue chair may not be reimbursed. Once adopted after the trial period, all future medical expenses will be the adopter’s responsibility.
- Foster Home agrees to hereby forever release, and covenant to hold harmless the DHRA, its members, representatives, officers, directors, Board members, and any other person, firm, or corporation charged or chargeable with responsibility or liability, from any and all claims, damages, costs, expenses, loss of services, actions and causes of action incurred as a result of this dog entering into Foster Care.
- This contract will apply to not only the dog in Section I but also any other future dogs affiliated with DHRA that are in my home. If a dog is transferred to or from my home for foster care, I will email medical@doghouserescueacademy.org to let them know where the dog is currently staying.
