Curricular Practical Training (CPT) is a benefit of F1 visa status. Before beginning employment, you must receive authorization from the Office of International Initiatives. Complete this form to request authorization. You will be notified once your new I-20 is ready to be picked up.

*Note: You must have a job offer before you can be approved for CPT. Make sure you have a copy of your employment letter before submitting this form.

Additionally, I understand that I may begin work only after I have received an updated form I-20 with CPT authorization from the Office of International Initiatives. The authorized employer, number of hours (part-time or full-time), and dates of CPT authorization will be listed on page two of my new I-20, and I will not engage in any employment that is not listed. If the need for CPT is based on enrollment in a course, I confirm that I have already registered for the course listed above.
