Mission Trip Dates:
Recommendation: avoid making travel plans for Sunday after return. Flights are subject to change and delays and those returning from mission trip need time to rest and recover.
Youth registering for one week may mark a preferred week, but placement in the preferred week is not guaranteed. Weeks are assigned in early February. After weeks are assigned, only those who can demonstrate bona fide conflicts or hardship will be considered for placement in the other week.
*** Zika Virus - is active in Colombia. Please consult your physician if you are pregnant or at risk of becoming pregnant before, during or soon after travel.
Meeting and Event Attendance:
Applicants are making a commitment to attend IMPACT meetings on Sunday evenings as scheduled. Please check your activites calendar and make sure you will be available the following dates (schedule subject to change):

Mandatory make ups for those absent from more than one evening meeting: Impact Brunches Scheduled
Mission fees cover personal expenses for the program and the trip. Additional funds will be raised to cover the cost of the projects we plan for the trip.
Participants should plan to bake or sell at the IMPACT Easter Sunday Bake Sale, April 21.
Participants will also be required to write fifteen (15) personal letters to friends and family to request sponsorship for mission projects in Colombia. Instructions for writing these letters will be shared at the March 10 meeting.
Church Volunteer Service:
10 hours total / at least 3 hours before registering.
Each program participant is required to complete ten(10) total hours Kenilworth Union Church volunteer service. Three (3) or more of those hours must be completed off-site. There are regular opportunities to serve at church on Sunday mornings and Wednesday evenings and there are often opportunities to serve on a flexible schedule. There are additional opportunities posted for sign up on the www.kucyouth.org website.
ALL program participants are required to connect to KUC Youth leadership on social media. This includes, but is not limited to snapchat, instagram, facebook, youtube, twitter, etc.
Participants are accountable for their live and online behavior and may have to be removed from the trip roster if they exhibit conduct that glorifies or associates with drug or alcohol intoxication, illegal use of substances, criminal acts, racism, anti-semitism, bullying, online harassment, violence, sexual promiscuity or exploitation.

EXEMPTIONS: CORE team, IMPACT Junior Board and IMPACT Worship Band members in good standing will fulfill service hours requirements through their respective commitments and are not required to log hours. If you are a musician and willing to commit to meeting and rehearsal times, you may proceed.

Permission and Liability Waiver
I have been advised about the nature of the program and consent fully to my child's participation in the Kenilworth Union Church IMPACT Mission Trip program. I understandthe program includes meetings, local service and other activities, in addition to a mission trip, which includes ground and air travel, lodging at a host location, traveling to different neighborhoods in and near Bogota, Colombia, and activities that may involve physical exertion such as lifting, carrying, balancing and working with tools and having contact with unsanitary conditions. Any of the included activities may pose the risk of injury, illness or death. The participant/guardian hereby release the Kenilworth Union Church and all of its agents, representatives, employees or service providers from any liability, claim, cause of action, or other expense or obligation whatsoever, that may arise out of or related to these activities.
Consent for Medical Treatment
I authorize Kenilworth Union Church adult leaders or agents to make decisions regarding medical treatment for the named minor in the case of illness, injury, or emergency, as they deem necessary, if a family member is not present or able to be reached to make that decision. I authorize the same to dispense over-the-counter medication to my child, as needed, for illness or injury unless noted otherwise in this child'e registration form.
I consent to the use of photographs and or videos of me/my child during the trip and related activities (no names are used to identify minors) by Kenilworth Union Church on social media, its website, annual report and/or other church-related purposes.
