AERIS Medical Aesthetics
This Membership Consent & Authorization ("Agreement") is entered into between AERIS Medical Aesthetics ("AERIS") and the undersigned client ("Member"). By signing below, Member acknowledges and agrees to the following terms and conditions of participation in the AERIS Inner Circle Membership Program.
1. Membership Overview
The AERIS Inner Circle is a monthly membership program designed to provide Members with exclusive access, priority scheduling, and rotating monthly perks that may be applied toward aesthetic services and treatments. Membership benefits are non-transferable and intended solely for the enrolled Member.
2. Membership Fee & Auto-Draft Authorization
Member agrees to a recurring membership fee of $199 per month.
Member authorizes AERIS Medical Aesthetics to automatically charge (auto-draft) the payment method on file on one of the following selected dates each month:

☐ 15th of the month
☐ 28th of the month
This authorization will remain in effect until the membership is cancelled in accordance with Section 6 of this Agreement. Member is responsible for maintaining a valid payment method on file. Failed or declined payments may result in temporary suspension of membership benefits until the account is brought current.
3. Monthly Perk Policy
Inner Circle Members may select one (1) signature perk per month. If a Member elects not to receive the included monthly service, they may instead choose a $100 AERIS Treatment Credit, which may be applied toward advanced services, upgrades, or injectable treatments at the time of booking. Credits are not redeemable for cash, may not be used without a scheduled service, and may roll over for one (1) month only unless otherwise specified. Monthly perks and credits may not be combined in the same month. All benefits are subject to availability and may evolve to protect the value of the Inner Circle program.
4. Treatment & Credit Limitations
• Membership perks and credits do not replace full treatment pricing unless explicitly stated.
• Credits may be applied toward services including, but not limited to, EVO Microneedling, laser treatments, RF treatments, and injectables.
• Injectable units, syringes, or medical dosing are determined at the time of consultation by a licensed provider.
• All treatments are subject to medical appropriateness and provider approval.
5. No Guarantees
Member acknowledges that aesthetic treatments may involve individual variation in outcomes. AERIS does not guarantee specific results. All services are provided in accordance with professional medical standards and applicable regulations.
6. Cancellation & Changes
Membership may be cancelled with 30 days written notice. Any prepaid or previously drafted membership fees are non-refundable. Membership benefits, pricing, and structure are subject to modification at AERIS’s discretion to protect program value and operational integrity.
7. Acknowledgment & Consent
By signing below, Member confirms that they have read, understand, and agree to all terms outlined in this Agreement, including automatic billing authorization and membership policies.
