    If you are a Florida Licensed Artist, we must have a copy of your license or your License number below.

  • BOOTH OPTIONS:

    • FULL 10x10 Booth - $625
      Two Artists, Four (4) Badges
      Each full Booth Space includes floor covering, two chairs, covered table (Plastic covers), electricity, sharps container, and bio-hazard disposal. Any other items needed are the responsibility of each exhibitor/artist. All artists and assistants must be listed above.

    • ADDT'L VENDOR WRISTBANDS - $35 each
      Limit to 2 additional per Booth

    All booths and tables will be reserved on a First-Paid, First-Serve Basis. If we don't have your payment prior to the event, you will not have a spot. We will NOT take reservations without payment/deposit, completed application and agreement to terms and guidelines.

    Deposit MUST be made within 2 weeks of receipt of application, or space will be forfeited!

    A 50% deposit is needed to hold your space.
    BALANCE due in full on or before March 8, 2019.
    Set up will not be permitted without full payment.

    Artists Licensed in the State of Florida:

    • Copy of your Florida Tattoo License, or at the very least your license number can be listed above

    Artists licensed outside of the state of Florida, please be sure to foward the following items to us via email or upload below along with your application.

    • Copy of your Drivers License
    • Proof of Completion of Bloodbourne Pathogens Training or State Licensure
    • Florida Department of Health Application for Guest Tattoo Artist Registration

  • Florida Bloodbourne Pathogen Training

    If you are licensed outside of the State of Florida, you will need to take this online course, a link can be found on our website under "Tattoo Festival" and here. The course for the State of Florida is $55

    Tattoo Artists must follow same rules and regulations as Exhibitors
  • 1. Payment
    a. Exhibitor agrees to pay all payments and fees listed above, and be contractually bound to make full and complete payment on the amount due.
    b. Exhibitor agrees that a fee of $50 will be charged for returned checks.

    2. Acknowledgement - I have read and understand the general terms and conditions and understand that filling out this application is not a guarantee of space. This application shall not be binding until this page of the application has been signed by both the Exhibitor and the Organizer.

    3. Authorized Signature - By signing below, I hereby acknowldge that I have read, understand and agree to be bound by both the General Terms and Conditions set forth below and the attached Rules and Regulations for Exhibitors (collectively the "Agreement"). I further acknowledge that failure to abide by the Agreement may result in the loss of my Exhibitor Privileges without recourse or refund.
