This questionnaire helps me learn more about you, your family, and the type of home you can provide. There are no right or wrong answers, please answer each question as honestly and thoroughly as you can.

This questionnaire takes approximatly 10 minutes to complete.

Completing this questionnaire does not guarantee a puppy. If I feel one of my puppies may be a good fit for your family, I'll contact you to arrange a phone conversation so we can get to know each other a little better.

2. Have you done research on the golden retriever breed? If yes what kind?
3. Why have you choosen the golden retriever breed?
4.How many members does your family have and what are their approximate ages?
5. Does everyone in the family want a puppy?

6.Do you have a fenced yard or dog run? If not how do you plan to keep your puppy from roaming?
7. Do you own your own home, rent or live with others? Please describe your living arrangements.
8. Does your daily schedule permit for daily exercise? Can you describe your lifestyle?
9. Is there anyone home during the day?
10. Does anyone in your family have allergies? If yes please describe.

11. Are you familiar with crate training? and do you intend on using one?
12. Are you willing to sign a non-breeding agreement?
13. Why do you want a golden retriever?
14. Do you have any other pets living in the home? If so what type of pet?
15. Do you forsee any changes in your life in the near future?
16. Are you interested in a female or male puppy? If your choice isn't available are you willing to take a puppy of the opposite sex?
17. If for some reason you are no longer able to keep the dog are you willing to return to the breeder?
