    An Online Application for Admissions must be submitted for each student in your family. Once you have successfully submitted one Application, you will have the opportunity to begin another application.

      To be considered for additional Financial Aid, a copy of your 2017 & 2018 IRS Tax Transcripts must be submitted to Combine Academy's Admissions Office. If currently available, please upload those documents below:

    • PAY STUB SUBMISSION

      To be considered for additional Financial Aid, a copy of your most recent pay stubs, dated within 30 days of the application date, must be submitted to Combine Academy's Admissions Office. If you are paid bi-weekly or twice-monthly, this will be two pay stubs, if you are paid weekly, this will mean we require four pay stubs, and if you are paid monthly, only one will be required.The submitted pay stub must be complete and the full document must be clearly legible. The name of your employer, your name, the pay date, and your deductions (social security, federal income tax, etc.) must be displayed, along with the net pay.     I understand that this application for admission will not be considered complete until my school transcript, proof of health insurance, official test scores (if applicable), non-refundable application fee, financial documents, and individualized needs documents have been received by Combine Charlotte, LLC.

    I aknowledge that the information presented on this application is complete, correct and truthful, and I am willing to abide by the rules and regulations set forth by Combine Charlotte, LLC. If anything is found to be untrue, Combine Charlotte, LLC reserves the right to revoke any offer of admission and, subsequently, remove the student from the Academy without refund.
