  • Praying Pelican Missions is based in the US. Therefore all payments to them are due in US dollars. Do you acknowledge and agree to paying all PPM related fees in USD?*
  • Payments to PPM are made as one team. Do you agree and acknowledge that the mission leader will make all PPM related payments on behalf of the team and that you will get your payments to her before their respective due dates so that she can do so?*
  • Do you acknowledge and agree to paying a NON REFUNDABLE $200 fee in USD to secure your place on the team by the above deadline?*

  • Deadlines are important and affect the team as a whole, as well as our relationships with our partners at PPM and in Puerto Rico. Do you agree to make every effort to meet deadlines that are asked of you?*
  • Do you acknowledge that if you do not meet the required deadlines, and as such our ministry partnerships or the team is negatively affected, you may be removed from the team without refund of prior payments.*
  • Mission trips can be expensive investments that require fundraising, do you have the time and determination to dedicate to fundraising?*
  • By checking "yes", you agree to obtain permission with your leader before beginning/promoting/soliciting any funds, if you choose to do so on your own or as a group (outside of organized full team events).*
  • By checking "yes", you agree that our Global Missions leader will keep record and possession of all full team funds raised. These funds will be held in your name and applied to the amount you owe when payments are due. For example: We raise $500 at a full team fund raising event. 10 team members participated in this event. The proceeds are split evenly at $50 per person. The CPGM leader keeps the $50 per team member and applies it to each team member's balance when payments are due.*

  • Mission trips are tremendous opportunities to be ambassadors for Christ and witnesses to the presence of God in our world. Participants must be willing to set aside personal preferences including such things as dress and lifestyle. Participants must be flexible in their deportment; adjusting their demeanor, posture and manner, as needed. Participants must be able to set aside comforts and conveniences that they may be used to. We will be spending a large portion of our time in prayer and service for the duration of our trip. By submitting this form, you are acknowledging & agreeing to this information.

    By submitting this form, you are also signifying that all of the information on this application is true to the best of your knowledge. You agree to abide by the rules and policies put in place by Center Point Church & PPM, and you understand that failure to do so may result in the termination of your trip at your own expense. You understand that mission trip planner staff would make this type of decision and you would abide by any decision made. You agree to sign a medical and/or liability waiver prior to leaving for your trip. You understand that you represent Center Point Church, Prince Edward Island and Canada while on this trip and will behave in a respectful manner at all times. 

    By submitting this form, you agree that our chosen mission organization Praying Pelican Missions & our Pastors in Puerto Rico will create a pre planned itinerary prior to leaving for the trip and you agree that you will adhere to this itinerary while on your trip. You acknowledge that changes may be made to this itinerary as needs & circumstances change, either before or during the trip. You agree to be flexible at all times, without complaints, if any changes come up.
