This information is used for the sole purpose of making you a new TU ID and the possibility of charging my bursar account. The information is stored on a secure server in the USA. This information will be shared with the Bursars Office and/or Human Resources. At anytime you have the right to ask us to remove your information. This provides us with compliance with the EU General Data Protection Regulation (GDPR). To have us remove your information from the server, you can email us at parking-cardservices@utulsa.edu.

If you do not agree for this information to be stored on this secure server, you will need visit Parking and Card Services office located in Fisher Hall.

Reason for requesting your Parking Permit Replaced (There is a $5 charge for permit replacement).

Permit Replacement $5 Charge
By requesting to replace or exchanging your permit, you agree to have your University account charged $5. This charge will immediately appear on your account.

Your parking permit replacement will be mailed to the adress you provided in this form. In the meatime, Parking and Card Services will email you a temporary parking permit. You will need to print this permit and place this on the inside, lower portion of the passenger-side, front windshield.
