1. Assemble and prepare equipment and supplies
Check the speed on the EKG machine. It should be set to the standard reading of 25 mm/second, unless you are
instructed otherwise. Make sure the unit is set to the full voltage.The unit will mark a standardization mark on the
paper. Next, enter facility-required patient identification information. When performing the EKG, if part of a wave extends beyond the paper, reduce the normal standardization to half standardization. Note this adjustment on the EKG strip.
2. Wash hands/don PPE (follow standard precautions)
3. Greet the patient
4. Identify yourself (name & designation)
5. Identify patient (full name & date of birth)
6. Explain procedure to the patient
7. Position patient comfortably
8. Prepare the skin for electrode placement:-

9. Apply limb leads:
Position the limb electrodes. Connect the lead wires. For easy visual identification, each is color-coded and lettered.
1) Right arm lead labeled RA
2) Right leg lead labeled RL
3) Left leg lead labeled LL
4) Left arm lead labeled LA
10. Apply precordial leads:
Apply electrodes to the chest. Avoid positioning the electrodesdirectly on bone, which will cause interference. In the female,position the electrodes below the breast tissue. If the breasts arelarge, you may have to position the breast laterally. Palpate the clavicle (collarbone), which is considered the first rib. Continuepalpating downward to the fourth rib. Move down slightly to thespace between the fourth and fifth ribs.
1) Position lead V1 in the fourth intercostal space, to the rightof the sternum.
2) Position lead V2 directly opposite V1 at the left sternal border.
3) Next, position lead V4 in the fifth intercostal space at themidclavicular line.
4) Position lead V3 halfway between V2 and V4
5) Position leads V5 and V6 laterally to V4
6) Position V5 in the anterior axillary line
7) V6 is positioned in the midaxillary line
11.Enter patient information into EKG as appropriate
a. After applying the electrodes, enter information required by the facility into the EKG Machine. Applying electrodes first allows the electrode gel to contact and penetrate the skin surface
12. Correct for artifacts as appropriate (Check)

13. Ask the patient to lie still and breathe normally
14. Select Print/Save on EKG machine to record the graph
15. Once complete disconnect the electrodes from the patient whilerespecting the patients privacy
16. Discard electrodes/contaminated items
17. Wash hands / Thank the patient
18. Document patient information and recorded graph
19. Place/Mount EKG Unit as appropriate (EKG Machine/Electrodes)
