The Commencement Absence Form is for students with an active graduation application for the current semester and who will be unable to attend their commencement ceremony.

**Please note that students are only eligible to participate in the commencement ceremony for which all graduation requirements have been completed. With the submission of this form, students are officially declaring their absence from the commencement ceremony of their eligible semester of graduation. Graduates may NOT participate in a commencement ceremony at a later time.

Please submit this form one month prior to the date of your scheduled commencement ceremony.
