This RSVP form is for our DACA renewal clinic ONLY on Friday, November 15th 2024.

RSVPs are required for each applicant renewing DACA. This form will automatically disable after SIREN reaches the maximum number of applicants we can assist for each clinic.

Please note that the appointments are phone appointments only. You DO NOT need to come into the office on the day of your appointment.

All appointments will average around 30 minutes. If you do not answer your phone when it is your appointment time, you may not receive another call during the same appointment date.

Once you have all of your supporting documents submmitted one of our staff members will contact you to fill out the immigration forms with you over the phone which the attorney will review on the date of our clinic and to confirm an appointment time for our clinic.

We would appreciate it if you could have all of your documents prepared and turned in by Wednesday, November 13th at 4PM.

The following documents are required.

- A copy of both your work permit and DL/ID front and back

- A copy of your passport (if not expired)
- A copy of your Social Security card
- Previous DACA application (if you do not have the previous one, you must have a copy of your first application)

-A copy of your most recent tax returns
- Court certified disposition (if you have ever been arrested)
