Please take a moment to complete this fabulous form to get your next staple design created for your next booking, pageant, or religious occasion. At the end you will pay your design fee, so our design team can start working on your fabulous creation(s). The $50 fee is non refundable, and your merchandise is available for 12 months to come back and repurchase on deejersdesign.online/designstaples/________________ (we will provide you with a special code for your line) and you can perform a standard checkout from the Deejers Design Online store! Turn around time for merchandise delivery is approximately 3-4 weeks so plan accordingly. Don't feel rigged if you can't plan for the gig!

And one last thing, check the sizes first. We are a screen print operation, not a factory of seamstresses. If you came looking for a gown, that's not us. If you came looking for jewelry, that's not us. If you came looking for the crown, that's not us, but we can help you get one of those.

Legal StuffBy clicking Yes below I attest that I will pay Deejers Design a commission rate to create a piece of artwork suitable for garments from a pre-determined collection made available by Deejers Design. At any time Deejers Design or I may terminate the commission and I will collect at minimum one master file per garment commissioned as selected above. This notice must be provided in an email communication to deejersdesign@gmail.com, and I will receive the files within 3 business days. It is understood that I have the right to use the images submitted above, and can upon request provide a photographer and/or model release for each image. Failure to provide documentation will result in a forfeiture of the commission and applicable removal of subsequent garments and items featuring said imagery. Deejers Design will not be held liable for any legal ramifications from use of images featuring trademarks or copyright material supplied through this request, and shall provide any parties seeking compensation the legal name of the individual supplied on this form. The commission fee includes the availability of purchasing merchandise online at www.deejersdesign.online. After 12 months online, the collection will be removed, and a reinstatement fee of $20 may be paid to Deejers Design to hold the image for three month periods, or at a rate of $5 per month on an autopay set up, paying $30 up front, for a final 6 months of online shopping availability. At any time Deejers Design may terminate the autopay and shall fulfill the agreed upon time that has been paid for appearance of a collection on Deejersdesign.online.
