Welcome to a wonderful journey to healing and freedom!  Deeper Still Upper Cumberland is a healing retreat specifically designed for women and men who have either had an abortion or who have participated in an abortion.  We affirm and encourage your decision to seek a deeper place of healing and restoration from your past choices. Our role is to help facilitate God’s healing touch in your life.

This is an opportunity for you to get away with God, and to receive His healing love.  There will be other women and men on a similar journey joining you this weekend. The ladies and men that serve on our Ministry Team are hand-picked and trained by us. Our women team members only minister to our women participants and our men team members only minister to our men participants. We can assure you that you can trust them all to be sensitive to the leading of the Lord, your needs, and your confidentiality.

When we receive your Release Form and Intake Form we will confirm your registration by sending you a confirmation packet via email. There is no registration fee for this retreat. Because our retreats fill up fast, we ask you to send in your registration forms ASAP. We will always maintain a waiting list in case we have cancellations. The confirmation packet will contain detailed information about the retreat location and what you should bring. If you will be driving and need directions, please let us know. If you will be flying, airport destinations are Nashville, Knoxville, and Chattanooga, Tennessee.

The intake form will take some time and thought to complete. You may find that some “old” emotions and memories surface as you fill it out. This is normal so do not be discouraged, but rather press through. These are simply indicators of places that may still need a healing touch. After we have sent out your confirmation letter and within 1-2 weeks of the retreat, we will call you to touch base and answer any of your questions.

Deeper Still Upper Cumberland is a program including healing retreats for women and men who have undergone or participated in an abortion procedure (the "Program").  The Program is specifically designed to address the emotional and spiritual wounds associated with abortion. 

Deeper Still Upper Cumberland retreats are hosted by a team of volunteers ("Team Members") who have received para-professional training to assist attendees in the healing process within the context of the Program.  Participation by attendees in Deeper Still Upper Cumberland retreats or any associated function(s) is not intended as a substitute for professional counseling and/or medical treatment.

By signing this agreement form, I freely and voluntarily agree with and understand the following statements to be true and I hereby agree to bind myself to such statements in consideration for the opportunity to participate as an attendee of an upcoming Deeper Still Upper Cumberland retreat.

• The Team Members are volunteers and not licensed professional counselors.
• The Team Members are not giving medical advice, making diagnoses, or providing licensed professional counseling.
• I will not hold Deeper Still Upper Cumberland, the Team Members, or other persons or entities directly associated with Deeper Still Upper Cumberland responsible for my actions or the actions of others made in response to any teaching, advice, ministry or any other goods and/or services I may obtain at a Deeper Still Upper Cumberland function.
• I fully release Deeper Still Upper Cumberland, the Team Members and/or persons/entities directly associated with them from any and all liability whatsoever.
• I assume full personal responsibility for any financial obligation I undertake based on and/or in response to any teaching, advice, ministry or any other goods and/or services I may obtain at a Deeper Still Upper Cumberland function.
• Information I give to any Team Member will remain confidential. However, I also understand exceptions to such confidentiality include:
  • any situation in which I communicate I am considering physically harming myself or another person.
  • any situation in which I communicate another person is continuing to or has expressed. his/her intent to physically harm me, and/or
  • any situation in which a formal grievance is brought against Deeper Still Upper Cumberland, the Team Members and/or persons/entities directly associated with them.
• Any formal grievance brought against Deeper Still Upper Cumberland, the Team Members and/or persons/entities directly associated with them are to be decided under Tennessee law and, if litigation ensues, in Tennessee courts of law in the Eastern District of Tennessee - the state and federal courts in which are deemed a proper venue by the undersigned for any such action - wherein such courts, based on this agreement, shall have personal jurisdiction over the undersigned. 

If any part of this agreement is deemed void by a court of law, the other portions of the agreement will remain in full force and effect.

If yes, we recommendthat you tell your counselor you will be attending this retreat. If you would like us to share information about our retreat with your counselor, we would be happy to do so. 

COVID Waiver

Introduction

The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing. It is recommended by the Tennessee Pledge that the groups follow social distancing guidelines and wear face masks.

Deeper Still Upper Cumberland has put in place preventative measures to reduce the spread of COVID-19 during the upcoming Deeper Still Upper Cumberland Weekend Retreat (the Retreat).   However, Deeper Still Knoxville cannot guarantee that you will not become infected with COVID-19. Further, attending the Retreat could increase your risk of contracting COVID-19.  Note:  Preventive measures shall include, but not be limited to:  1) hand-sanitizer and disinfectant wipes shall be made readily available; 2) masks and gloves available for those who desire to utilize them; 3) particular efforts to provide social distancing during hours of sleep; 4) careful attention to handling of meal prep and service; 5) a brief symptom evaluation will be taken upon arrival and possibly during the Retreat.

Assumption of the Risk & Waiver of Liability Relating to Coronavirus/COVID-19

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending the Retreat and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Retreat may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Deeper Still Upper Cumberland team members and guests.

By signing below I also agree that in the last 21 days I HAVE NOT: 1) tested positive or presumptively positive with the Coronavirus nor have I been identified as a potential carrier of the Coronavirus; 2) experienced any symptoms commonly associated with the Coronavirus; 3) been in any location positively designated as hazardous and/or potentially infected with the Coronavirus by a recognized health or regulatory authority; 4)been in direct contact with or the immediate vicinity of any person I knew and/or now know to be carrying the Coronavirus or who has been identified as a potential carrier of the Coronavirus.

I AGREE to notify Deeper Still Upper Cumberland (by email to Chapter Leader, Lisa Mackie @ lisa@deeperstilluppercumberland.org) of any change in status, including diagnosis with Coronavirus and/or quarantine, within thirty (30) days either before or following the Retreat.  I also AGREE to consent to having my temperature taken by a Deeper Still Upper Cumberland representative upon arrival and possibly during the Retreat. In becoming symptomatic (fever) I will agree to leave the retreat facility, and seek medical attention immediately.

I ACKNOWLEDGE & ACCEPT that this Declaration shall be governed by the laws of Tennessee.  I irrevocably agree that the competent Courts of Tennessee shall have jurisdiction to hear and determine any suit, action or proceeding, and to settle any dispute which may arise out of, under, or in connection with this Declaration and for such purposes hereby irrevocably submit to the jurisdiction of such Courts.  Nothing contained herein shall limit the right of Deeper Still Upper Cumberland to take proceedings in any other Court of Competent jurisdiction nor shall the taking of proceedings in any other jurisdiction whether concurrently or not one or more jurisdiction preclude the taking of proceedings in any other jurisdiction whether concurrently or not.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at the Retreat. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Deeper Still Upper Cumberland, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Deeper Still Upper Cumberland, its volunteers, employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Deeper Still activity.
