Photo Indemnity Permission Form 2026

Please read the information below carefully and comment below in the box if you do NOT give photo release permission. Those who do give permission please sign this form. Students of Bayside Ballet Academy may have the opportunity to be photographed or filmed in class or at various other in house or external dance events. With permission from parents or adult students these photos or video may be included in BBA publications or advertisements, including on our website and on Social Media. To ensure privacy, full names of students will not be included. No financial compensation shall be given for use of any photographs or video. I acknowledge that BBA may chose not to use my photo or video at this time but may do so at its own discretion at a later date. BBA reserves the right to discontinue use of photos or video without notice. I have read this release and I understand its contents. I consent to and authorise the use of BBA, or anyone authorised by BBA, of all photographs or video which have been taken of my child/children. If you prefer not to grant permission for this, please comment in box below and write 'no permission given ' and if permission is granted please sign or write your name and the date below. This form expires on the 31st of December 2027 Thank you.
