MODEL RELEASE FORM
1. I contract that, by sitting and/or being photographed for the photographer indentified below, I grant the photographer absolute right and permission to publish, sell, or use any and all photographs or images made of me at that time.
2. I undertake not to prosecute or to institute proceedings, claims or demands against the Photographer, the Town of Halton Hills or "PhotoArt Photography Competition & Exhibition" in respect of any usage of the above-mentioned photographs. I hereby release the photographer named below from all claims and liability relating to images or photographs taken of me..
3. I hereby waive any right that I may have to inspect and/or approve the finished product of the use to which it may be applied.
4. I hereby warrant that I am of full legal age and have every right to contract in my own name in the above regard. I state that I have read the above authorization and release prior to its execution and that I am fully familiar with the contents thereof.

Entries submitted without model release forms will be disqualified.
