  • INSTRUCTIONS - PLEASE READ

    • Inputs with a red asterisk * indicate a response is REQUIRED.
    • To move from one entry field to the next, it is best to use the mouse and click into the entry field.
    • To navigate between pages, use the BACK and NEXT buttons at the bottom of the page. The SAVE and EXIT button can be used to save your current entries and return later to finish (a link will be emailed to you that will allow you to return to your partially completed form).
    • There is a general comments area at the very end (before submission) where you can enter any questions or additional information that you couldn't enter anywhere else on the form.
    • Click the SUBMIT FORM button when you have completed the form to your satisfaction.
    • You will receive a confirmation email after submission with a PDF attachment that you can download and/or print.
    • The confirmation email includes a link (at the very bottom) which will allow you to return to this form if you would like to make edits or additions.

  • Facilitator/Presenter Information



     

    • Choose FACILITATOR if you are holding an actual class that will meet at least twice (2 sessions) OR are facilitating an off-site field trip.
    • Choose CO-FACILITATOR if you are co-facilitating a class or off-site field trip with another facilitator (and he/she has already completed a full proposal form as the main Facilitator).
    • Choose ONE-TIME PRESENTER if you are doing a single presentation such as for the Friday Speaker Series or a Lunch And Learn noon-time presentation.
