I, the participant and signee of this agreement, assume all risks associated with participating in physical activity provided by Kyle Impero Hoops-Elite Training, including, but not limited to basketball training, endurance and conditioning activity, weight lifting, rowing machines, conditioning/weight machinery , falls, contact with backstop, contact with other participants, building and weather hazards, playing surface, basketball competition, including, but not limited to 3-on-3 tournaments and leagues, basketball camps, etc. I fully understand that participating in Kyle Impero Hoops-Elite Training can bring injury upon me. I hereby for myself, my heirs, executors, administrators or anyone else who might bring claims on my behalf agree not to sue, and waive, release and discharge any and all individuals, owners, of KI Hoops-Elite Training from any and all claims of liability of death, personal injury, or property damage of any kind arising out of or in the course of participating in Kyle Impero Hoops-Elite Training physical activity services and competitions.
I know of no medical or physical problems which may affect my ability to safely participate in any Kyle Impero Hoops-Elite Training programs and events. In the case of a medical emergency, I authorize the owner, directors, employees, volunteers to put forth their best efforts and judgment requiring medical attention. Additionally, if my health is to change and affect my physical activity, it is my responsibility to let the owner of Kyle Impero Hoops-Elite Training know, as well as seek medical advice from a professional, ensuring participation at Kyle Impero Hoops-Elite Training is right for me.
Inconsideration with my participation associated with Kyle Impero Hoops-Elite Training, I, for myself, my heirs, executors, administrators or anyone else who might bring claims on my behalf here by holds harmless Kyle Impero Hoops-Elite Training, its owners, individuals, employees, agents, from any and all claims, demands, in causes of action, to include reasonable legal expenses and attorneys fees arising from my participation with Kyle Impero Hoops-Elite Training. I hereby grant full permission to any and all foregoing to use photographs, videotapes, motion pictures, recordings, or record me or my likeness from this event for any purpose associated with KI Hoops-Elite Training.
I hereby confirm that I have read the above and fully understand the above information. I confirm that I have been honest with Kyle Impero Hoops-Elite Training when filling out any information, and when considering my health. If I am under the age of 18, I understand I am to have a guardian read and sign this waiver as well. I have been given the opportunity to present questions in matters related.
By signing below, I acknowledge the following:

My participation in services offered by Kyle Impero Hoops-Elite Training is completely voluntary, and I understand participation in such services does not guarantee the achievement of my basketball and fitness goals;

I acknowledge that physical risks are a possibility, however I believe that the benefits of Kyle Impero Hoops-Elite Training programs are greater than the possible risks;

I understand that certain physical touching, as is specific in respect to the drill and/or exercise, may be necessary for the demonstration and correction of technique;

I have been provided time to ask questions and receive answers from Kyle Impero Hoops-Elite Training;

In correlation to any medical problems, it has been recommended that I consult with my physician;

I have no known health conditions that would stop and/or impair me from participation in physical activity; if I am to experience discomfort or have a change in medical conditions, I am to let the owner of Kyle Impero Hoops-Elite Training know immediately.
