• DPT Junior Early Assurance Pathway (JEAP) Application

  • Important Dates:

    March 15th - Submit the Junior Early Assurance Pathway Recommender Form

    April 1 -        JEAP Application Due

    April 7 -       Recommender Evaluations/letters deadline to submit

    Applications will not be reviewed until all documents have been submitted to the admissions committee.  Please note that once your application has been submitted, no additions or changes can be made.

  • 3. Observation Hours:

    Observation hours for the JEAP application process are not required, though the hours must be completed by October 1st in the final undergraduate year when you are completing the PTCAS application. If the clinical site does not provide a form you can utilize the form provided on the UC DPT Application Requirements webpage. Please note: You will need to submit either a signed form OR require an electronic signature from the clinical site. Self-reported hours are not accepted in PTCAS by UC.

  • 6. Letter of Recommendation:

  • You are required to submit ONE letter of recommendation using the DPT JEAP Recommender Form.  The letter should be from a physical therapist who supervised your observation hours or someone who can attest to your character and/or desire to become a PT. 

    Please indicate below the individual that you submitted in the DPT JEAP Recommender Form. As a reminder, the recommender form needs to be completed in advance.  Deadline:  March 15th

    Please forward an unofficial or official transcripts for all colleges and/or universities attended. University of Cincinnati transcripts can be unofficial versions downloaded from your Catalyst account. Even if none of your former college or university credits transferred to UC’s health sciences program, you must still submit a transcript from that college/university and list them in the table above.
