1. Please send the reference link to three references: 1) one professional, 2) one personal, 3) one family member. 
2. References must include one family member and one member of the opposite gender and can include members of the church.
3. Responses to the reference form will be received and retained by an authorized CFC staff member and handled by them and the pastoral team as appropriate.
