INSTRUCTIONS
Complete and sign this application, and submit the following documents:
1. A written or typed personal essay about yourself, and your interest in the skilled trades. You may include descriptions of classes you’ve taken, projects you’ve worked on, or previous and current work experience. A blank page is provided in this packet for you.
2. Two or more references, using the forms provided in this application. One reference must be a teacher (Adult students having been out of school longer than 5 years: if a teacher is not available, please use a mentor or similar for this reference). The others may include other teachers, school staff, an employer, or other professionals you know personally. If you’d like to include more than three references, please make copies of the reference forms in this packet.
