    • LifeBridge Health and Fitness Membership Agreement

    • The terms "you" and "your" in this Agreement refer to the Member and the additional Member(s) (if any). Pending receipt and continued timely payment of all applicable fees, and subject to the rules and regulations set forth herein, LifeBridge Health & Fitness Center (the "Center") admits you to membership in the Center for the period of {startDate} through {endDate} (the "Initial Term"), and grants you the right and privilege to use the Center’s facilities, in accordance with the terms of the Agreement.

      1. Enrollment Fee. THE ENROLLMENT FEE IS A ONE-TIME FEE of {enrollmentFee}, as long as you remain a member. It is payable upon the execution of this Agreement and is subject to your payment of membership dues and the other terms of the Agreement. The Enrollment Fee entitles you to membership in the Center for the Initial Term. If your membership lapses for any reason, the current enrollment fees apply to rejoin. Except as expressly provided in the Agreement, the enrollment fee is not refundable.

      2. Payment Arrangements. A monthly payment of {monthlyPayment} will be taken on the first business day of each month beginning with the month of {beginningMonth} and will continue as long as this authorization is in effect.

      Pro Rate due with this Agreement {proRate}.

      If your bank refuses to honor a withdrawal because there are insufficient funds in your account, or your credit card issuer refuses to honor the charge for any reason, you will be liable for an additional $15 reprocessing fee. You may make changes to this authorization at any time by giving notice to the Center’s Business Office at least thirty (30) days before the change is to take effect. Changes to this authorization will not relieve you of any of your outstanding obligations under this Agreement, including, but not limited to, the obligation to pay membership dues.

      3. Renewal of Membership. After the Initial Term, your membership will automatically continue on a month-to-month basis at the then prevailing rates unless the agreement is renewed for another 12 months. On January 1st of each calendar year, your membership rate is subject to a maximum increase of 5%.

      4. Young Adult Membership. The Center offers a discounted rate for adults under the age of twenty-five. When you reach the age of twenty-five, your monthly dues will be adjusted to the then-current individual membership rate. You may not extend your contract at this rate after turning twenty-five years of age.

      5. Members Right to Cancel. You may cancel your membership at any time after the Initial Term by giving the Center’s Business Office thirty (30) days advance notice of cancellation. Notice of cancellation must be in writing and must be delivered in person, by fax or e-mail, or by registered or certified mail, with a return receipt requested. The Center will not provide retroactive refunds for members who have failed to give appropriate notification. Verbal notice of cancellation is insufficient. Your membership fee will be forfeited upon cancellation except as provided in the notice of consumer rights section. All outstanding fees must be paid upon cancellation. An early termination fee will assessed on any contract cancellations except month-to-month members or as provided in the notice of consumer rights herein. Reversion Clause: If the principal member cancels his/her membership and does not simultaneously cancel additional members under this Agreement, the dues rate of the non-minor, second member on the membership will revert to the dues rate of the primary member. The rates of any other secondary members on the agreement may be subject to change.  {initials} Member Initials

      6. Termination. The Center reserves the right to terminate your membership immediately if you fail to comply with the Center’s rules and regulations, if you engage in other conduct that is detrimental to the health or safety of the staff, other members, or guests, or if you fail to pay membership dues or other amounts that you owe to the Center. Upon termination or cancellation of your membership, you will remain liable for any outstanding dues or other charges owed by you.

      7. Membership Freeze. You may obtain a medical or voluntary freeze of your membership under the below circumstances:

      a. Medical Freeze. If you are medically unable to exercise, you must notify the Center and provide medical documentation from your physician to the Center’s Business Office. The freeze will be effective as of the date the complete documentation is received requesting the medical freeze. The freeze must be for a minimum of 1 month. After a 6 month freeze, the freeze may be renewed with updated medical documentation from a physician. The freeze period does not constitute fulfillment of any part of the initial 12 month membership agreement. No retroactive refunds will be issued. Written clearance from your physician is required to be provided to the Center upon return from a medical freeze. A $10.00 per month fee will be charged during the period of your medical freeze. 

      b. Voluntary Freeze. You may freeze your membership voluntarily for any reason. Written notice must be given to the Center’s Business Office 30 days prior to the freeze date. The freeze must be for a minimum of 1 month and a maximum of 6 months. The freeze period does not constitute fulfillment of any part of the initial 12 month membership agreement. No retroactive refunds will be issued. A $30.00 per month fee will be charged during the period of your voluntary freeze.

      c. Temporary Residence Freeze. If you maintain an out of state residence and will be temporarily out of town for an extended period of at least 3 months but no more than 6 months, you may freeze your membership by providing proof of out of state residence. Written notice must be given to the Center’s Business Office 30 days prior to the freeze date. No retroactive refunds will be issued. A $15.00 per month fee will be charged during the period of your voluntary freeze.

      8. Relocation. In the event you should relocate outside a 25 mile radius of the facility, you may cancel your membership by giving us thirty (30) days advance notice by certified mail. Proof of relocation is required. Membership ID card must be surrendered at the time of cancellation and all unpaid charges must be current.

      9. Operating Procedures.

      a. The operation and management of the Center will be under the Center’s complete control. Resolution of all matters regarding the Center, including, but not limited to, membership classifications, hours of operation, use of facilities and equipment, guest policies, and termination of membership, shall be at the absolute discretion of the Center. You agree to comply with the Center’s rules and regulations, which are subject to change at the discretion of the Center. Your use of the Center’s facilities may be interrupted from time to time for repairs and maintenance.

      b. The Center reserves the absolute right to initiate, change or eliminate facilities, services and programs offered from time to time; to initiate, change, or eliminate fees for existing or new facilities, services and programs, and to change its hours of operation.

      c. By your signature on this contract, you acknowledge receipt of a copy of the Rules and Regulations of the Center and agree to abide by those terms, all terms in this contract, and all other rules/policies that are either posted at the Center or provided to you directly.

      10. Member Responsibilities: You agree that, prior to undertaking any physical activity at the Center, you will advise your personal physician of the details of the activities in which you plan to participate and will obtain your physician’s approval of those plans. You also agree to consult a physician and reconfirm your physician’s approval if any injury or other problem or condition arises that may affect your ability to participate in the activities offered by the Center, or in the event of any new illness, injury, discomfort, or other health problem.

      In addition, you acknowledge that:

      a. The physical fitness programs offered by the Center and any health risk appraisal provided by the Center are not substitutes for regular medical checkups, proper diet, or other activities related to good health maintenance.

      b. Participation in physical fitness activities, including those available at the Center, may be physically stressful and may result in illness or physical injury.

      c. The services offered by the Center are not those of a physician or medical treatment facility and should not be used as a substitute for those services.

      d. Theft and Liability: It is further expressly agreed that all exercise and treatment and the use of all facilities shall be undertaken by the member at his/her own risk and that the Center will not be liable for any claims and causes of action of any kind or nature which are in any way related, directly or indirectly, to the use of any of the services, equipment, or facilities provided at the Center, including any such claims or causes of action caused in whole or in part by the negligence of the Center, its subsidiaries and affiliates, and their respective agents, employees, officers, directors, successors and assigns. This agreement applies to the use of all facilities and services of any kind, including, but not limited to, the use of the KidZone. The Center will not be held responsible for the loss or theft of a member’s property.

      e. Age Restrictions and Children: Members and guests must be at least 14 years old. Children between the ages of 2 months through 13 years must be supervised in the KidZone by the Center staff when the service is provided. All KidZone rules and policies apply.

      11. Assignment and Transfer. The Center may assign any or all of its rights and obligations under this Agreement. You may not transfer your membership in the Center under any circumstances, nor may you lend your membership card to anyone for any purpose.

      12. Notice of Consumer Rights. You are entitled by Maryland law to the following information.

      a. The Center is registered with the Consumer Protection Division, Office of the Attorney General of the State of Maryland. The Center’s registration number is 12674.

      b. The Center’s obligations to you are secured by a security bond in the amount of $200,000. The amount of the bond may be increased or decreased from time to time in accordance with the requirements of Maryland law.

      c. You are entitled to a copy of this Agreement. You have the right to cancel this Agreement within three business days after receiving your copy. To cancel the Agreement, you must give the Center written notice of cancellation. Notice of cancellation under this provision may be delivered to the Center at 1836 Greene Tree Rd. Baltimore MD 21208. In person, by fax or email or may be sent to that address by certified or registered mail, return receipt requested. A notice sent by mail must be postmarked by midnight of the third business day after the day on which you receive your copy of this Agreement. If you cancel within three business days of receiving your copy of the agreement, you are entitled to a full refund of the initial membership fees paid. Any unpaid charges to the club must be made current.

      d. If the Center is completely closed for one month or more due to fire, flood, or other event that is out of the Center’s control, the Center will, at its option, either (1) extend your membership for a period equal to the period during which the Center is closed, or (2) refund your membership dues for that period (and, if the closure occurs during the initial term, a prorated portion of your enrollment fee). If the Center is completely closed due to the fault of the Center for one month or more, you may, at your option, either (1) extend your membership for a period equal to the period during which the Center is closed, or (2) receive a refund of your membership dues for that period (and, if the closure occurs during the initial term, a prorated portion of your enrollment fee). 

      13. Membership ID; Photograph. You will receive a membership ID barcode card (the "Card"). You are required to present your Card at the front desk upon entering the Center. If your Card is lost or stolen, you must notify the Center immediately. Your Card may be replaced for a nominal fee. Misuse of a Card or permitting another person to use your Card will be cause for termination of membership, without a refund. For security purposes, a photograph of each member is maintained by the Center.

      14. Release. By participating in activities at the Center, you may be photographed or videotaped. By your signature below, you consent, permit and authorize the Center and its affiliates, agents, employees, contractors, personnel and others acting on the Center’s behalf, to photograph, record and use your likeness, voice and image in drawings, photographs, videotapes, sound records, motion pictures or other media materials (collectively, "Media") which may be used for promotional, research or other purposes reasonably deemed appropriate by the Center and you acknowledge that all such Media will be the sole and exclusive property of the Center.

      15. Entire Agreement. You acknowledge that, except for what is contained in this agreement, the Center has not made any promises, representations, or warranties to you. The terms of this Agreement may be amended only by a written instrument signed by both the Member and an authorized employee of the Center. By signing below, you acknowledge receipt of a copy of this Agreement and agree to be bound by its terms.

      Group Exercise

      Group classes combine the energy, motivation and fun of exercising with others to the guidance and safety of working with a professional fitness instructor.  Whether you’re in the mood to build or sculpt muscle, burn fat and calories, stretch and strengthen or dance we’ve got the right classes for you.  The club offers 3 state-of-the-art studios with numerous classes each week to choose from.

      Studio A offers a wooden spring floor with the finest sound system and large mirrors with classes from Zumba to BodyPump to kickboxing and total body conditioning.

      Studio B is a mind body studio with mood lighting and temperature control with highly trained instructors offering classes at all levels including classes that are gentle, vigorous, vinyasa yoga as well as yoga workshops on a variety of themes.

      Our Cycle Studio has 26 bikes with over 20 cycle classes and theme rides.

       

      Personal Training

      Everyone who is serious about achieving his or her goals should consider personal training.  Our degreed/certified trainers will provide the knowledge, motivation and instruction you need to achieve your health goals.  The trainer will record your progress and make recommendations to keep you moving towards better health.  Use a personal trainer once a month or every week-it is up to you.  We are confident that you will be pleased with the results.  Train one-on-one, buddy training or in a small group.  Please ask a Fitness/Personal Trainer for more details.

      Note: All personal trainers must be employed by LifeBridge Health & Fitness to conduct training sessions with in the facility.  Using a personal trainer or having the appearance of using a personal trainer, that is not an employee of LifeBridge Health & Fitness, may result in suspension or termination of your membership.

       

      Pilates

      STOTT PILATES done regularly either on mat or using specially designed resistance equipment-improves posture, muscular strength, flexibility and endurance, without adding bulk.  In our fully equipped STOTT PILATES studio, we have state of the art STOTT PILATES equipment.  Our certified STOTT PILATES instructors run small group training, or private sessions.  We are the only STOTT PILATES Licensed Training Center in the area, offering Pilates education and workshops throughout the year.  Additional fees apply.  For more information, please contact the Pilates Team Leader at 410-318-6831.

      Face2Face New Member Journey

      Welcome to LifeBridge Health & Fitness, Maryland’s premier fitness and wellness facility.  We are glad you have chosen to embark on your fitness journey with our team of professional fitness experts.  We recognize that each member has unique fitness needs so we take a serious interest in your fitness goals.  Our aim is to not only meet your needs, but to help your realize your maximum fitness potential in order to lead a healthier life.

      You’ll experience the best start possible with our F2F program.  This fully structured induction program caters to your individual needs as you are guided by our Fitness Trainers who will offer feedback and fitness empowerment to lead you toward a healthier lifestyle and help you establish life-changing habits.

      Not only will you get off to a great start, but you will stay engaged through the interactive, educational and motivating programs and systems that F2F provides.  Members may take advantage of a Program Review every 6 months.

       

      Checking In

      • Members and guests must check in at the Service Desk by use of their membership card.
      • Membership cards are unique to each member and are not transferable.
      • You may be required to show additional photo ID prior to admittance to the club.
      • Account must be in good standing in order for any member to use the club.

       

      Membership ID Cards

      • Membership cards are provided to members and required for entry into facility.
      • A $2 fee will be charged to replace a lost card or key fob.

       

      Towel Service, Locker Room and Lockers

      • Towels are provided at no additional cost.
      • Children under 14 are not permitted in the locker room area, unless it is during designated times for swim lessons or family swim night.
      • Lockers are for daily use only.  Articles are not to be left in lockers overnight.  The club reserves the right to remove contents left in an unrented locker overnight.  .
      • The club is not responsible for lost or stolen articles.  If it is necessary to bring valuables into the club, complimentary lock boxes are available for daily use.

      Parisi Speed School

      • From athletes 7 years old all the way to the Pros, the Parisi Speed School has allowed athletes to do the two things our program strives for: improve athletic performance and more importantly, develop a higher level of self-confidence.  Focus on injury prevention, agility and speed training to maximize your natural abilities.

      IHRSA

      • IHRSA is not for profit trade association for owners and managers of fitness and racquet facilities throughout the USA, Canada and 40 other countries.  Currently, over 3000 health clubs belong to IHRSA, making it the largest club association in the world.

      • You will present your membership card when visiting reciprocal clubs.

      Pro-Shop

      • For your convenience, LBHF provides an assortment of healthy snacks perfect for replenishing essential nutrients after a workout.  We also offer clothing items from UNDER ARMOUR and other fitness products at discounted prices.  For your convenience, house charge is available for members with a current EFT.

      Business Center

      • The business center is located in the lobby of the club and consists of (4) four computer stations each with a telephone.  Please use the center as needed, but limit computer time and phone time to 15 minutes so others may take advantage of this resource. 

      Security of Your Personal Belongings

      • LBHF is not responsible for any belongings lost or stolen at the club or in the surrounding areas.  We do, however, have a recovery area for any item that has been lost and turned in.  Please notify the Service Desk staff member if you have lost an item.

      General

      1. The club reserves the right to amend any rules, policies or hours of operation at any time.
      2. Members and guest must check-in at the Service Desk.  Members must have their ID cards.  All guests must be registered at the Service Desk and pay the appropriate guest fee.
      3. Children 12 and under must be checked in the KidZone and may not be left unattended in the club.
      4. Shirts and closed-toe shoes are required at all times while using any exercise equipment and/or free weights.  Bare feet are not allowed in the club (excluding aquatics area)
      5. Smoking is not permitted in the club or within 20 feet of any club entrance

      Contact and Account Information

      It is the member’s responsibility to notify the club of any change in address, phone number, e-mail or billing information in order to maintain a current account.  See the Service Desk to complete a Status Change Form.

      Language and Conduct

      LifeBridge Health & Fitness will not tolerate any obscene, derogatory or abusive language, either from staff or members.  We will not tolerate any conduct, verbal or physical, that can be in any way interpreted as offensive, intimidating or sexually harassing.  All members are expected to communicate with dignity and mutual respect.  Violation of this policy may result in suspension or termination of your membership. 

       

      Guest Policies

      • Non-members are welcome at the club for an appropriate guest fee.
      • It is the responsibility of the member to see that their guests are properly registered and the appropriate guest fees are paid.
      • Members are responsible for the conduct of their guests.
      • Guests ages 14-17 are allowed only when accompanied by an adult.

      Membership Freezes

      For your convenience with proper notification and/or documentation, we can “freeze” your membership temporarily.  We offer 3 types of freezes.  Please note that different rules apply for each:

      1. Medical Freeze
      2. Voluntary Freeze
      3. Temporary Residence Freeze

      Membership Cancellation

      1. Memberships renew automatically after the initial term.
      2. A member may resign their membership with minimum 30 day written notification through the approved cancellation form.
      3. Resignation from the club does not relieve the member of the responsibility to pay all applicable charges prior to expiration of the 30 day period.
      4. A member wishing to cancel before the obligation date may “buy-out” the remainder of the membership for a $150 fee per person and $10 per remaining month agreement per person.
      5. The club reserves the right to terminate any membership or usage at any time at its sole discretion., for any reason, but not limited to:
      • Non-payment of member account.
      • Failure to comply with the club policies and procedures.
      • Aggressive, disruptive or abusive behavior and comments.
      • Any action that interferes with the operation of the club or members enjoyment of the facility.
      • Incomplete membership application, including but not limited to all necessary signatures, proof of residency and valid payment information.
      • All membership suspensions or terminations for violation of club policies are at the sole discretion of the LifeBridge Health & Fitness Executive Director.

      Payment for Services

      1. Monthly membership dues and any outstanding house charges from the previous month are automatically billed to your chosen method of payment on the first day of each month for that month’s dues.
      2. The club accepts Master Card, Visa, Discover, and American Express.  A credit card or checking account is required for monthly billing to maintain membership.  Only one method of payment may be used on an account.  Members may also pre-pay for membership annually.
      3. Unpaid accounts will be assessed a $15 per month fee until balance is paid in full.  Returned checks are subject to a service fee.
      4. Returned credit cards are subject to a minimum $15 service charge.
      5. Add-on members may bill incidental charges to their account for additional facility services and/or merchandise.  Primary members are responsible for payment for all incidental charges.
      6. Members may establish house-charging privileges through the use of their personal credit card or checking account.
      7. To make changes to account information, a Status Change form must be completed.  See the Service Desk.

      Program Registration Policies

      • Participants currently enrolled in a class will have first priority to register for the next session of that class.  This registration and payment must occur 3 weeks prior to the class starting.  Club members must register 1 week prior to the class starting.  Non-member registration for those not enrolled is based upon space availability.
      • Participants may register at the Service Desk or with an instructor for the specific program.  Registration forms must be completely filled out and signed.
      • Payment in full is due upon registration.
      • A minimum number of participants may be required for each class.
      • Credit Policy: All program registration fees are non-refundable.  In case of serious injury or illness, please see the Manager/Team Leader of that Department.
      • Pro-rate Policy: All late entries into the existing program may be pro-rated for the remainder of the session in progress only if approved by the Departments Manager/Team Leader.
      • Make-Up Policy: There are no credits or refunds for missed classes, nor are there pro-rates for future sessions if a make-up is not scheduled.  With the advanced notification that a class will be missed, we will try to accommodate you in another similar class based on availability.  Missed classes must be made up within the session they are missed.  Make up classes do not carry over from session to session.
      • Children under the age of 14 must be escorted to and from classes by parent/guardian or instructor.  Parents are welcome to observe classes without causing a distraction or obstruction.  Younger siblings that are not enrolled in the class are not permitted in the activity area or on any equipment.  Non-member children may be checked into the KidZone based upon space availability and provided they have paid the appropriate fee.  To make arrangement for adults other than parents to view a particular class, please contact the Manager/Team Leader of that Department.
      • Non-members will be given a non-member ID card and must check in at the Service Desk.  This card permits the person into the class only; it does not grant permission to use any other part of the club’s facilities.
      • Non-member participants may enter the club 15 minutes before a scheduled program.  All non-member participants must exit the building at the conclusion of each class.  If you wish to work out before or after your program, you must pay the appropriate guest fee at the Service Desk.  You will be expected to adhere to all club policies and failure to do so could result in termination of your eligibility to participate in the program or future programs at LBHF.
      • The club reserves the right to cancel or consolidate any class or activity.

       

      Health Warranty:  Participants warrants and represents that he/she has no disability, impairment or ailment preventing him/her from engaging in active or passive exercise or that will be detrimental or inimical to hi/her health, safety or physical condition if he/she does so engage or participate.  This representation is made by participant knowing that management will rely upon same in respect to the registration of participant.

       

      Liability and Waiver of Liability:  Participant accepts full responsibility for his/her use of any and all equipment, appliance, facility, privilege or service whatsoever, owned or operated by this organization or any other activity organized or sponsored by this organization on or off the facility’s premises at his/her own risk and shall hold this organization and its directors, officers, employees, representatives and agents harmless from any and all loss, claim, injury, damage or liability sustained or incurred by him/her resulting there from.

      Fitness Floor

      1. You must be 14 years of age to be on the fitness floor.
      2. Personal items must be stored in lockers and not on the floor, countertops or railings.
      3. Appropriate clothing and closed-toe shoes and shirts are required.
      4. Food in not permitted on the fitness floor.  Only non-spill drinking containers are permitted on the fitness floor.
      5. All users must re-rack their own weights as a courtesy to other members and guests
      6. Members and guests are responsible for the wiping down of their equipment after each individual use.  Wipes are available throughout the fitness area.
      7. Members and guests are asked to place towels in the towel bin after use.
      8. All members and guests are asked to use common courtesy when using the equipment (i.e. allowing another member to work in).
      9. Members are not allowed to remove equipment from the fitness floor.

      Whirlpool, Steam, Sauna

      1. No children under the age of 14 years may use these amenities.
      2. Please abide by posted rules.
      3. Temperature of whirlpool, steam room and sauna are all in accordance with the American College of Sports Medicine guidelines.
      4. Bathing attire should be worn when entering the whirlpools.
