Students participating in extracurricular activities on a regular basis outside regular school hours may request to receive high school credit. Some examples include music lessons, team sports, foreign language, etc.
Please read the following qualifications BEFORE completing and submitting this application form:
- Liahona reserves the right to determine course credit eligibility.
- Liahona will not grant credit for a class currently offered in our curriculum.
- The course teacher may not be a parent or relative.
- All off-campus credit requests must be approved before beginning any coursework.
- The student must be able to provide sufficient evidence as requested. Liahona Administration determines all supporting evidence/documentation.
- Students are required to keep a log of participation hours, which will be submitted with required coursework.
- The teacher must grade each course. They will be emailed a link to a grade report form, which they must complete prior to Liahona awarding course credit.
- Earned credit is based on hours of participation as follows: 45 hours =.25 credit, 90 hours=.5 credit 135 hours=.75 credit 180 hours=1.0 credit
- Fees: $100.00--up to .5 credit $200.00-up to 1.0 credit
