My Responsibilities:

• I will pay my deposit or PAY IN FULL for my service at the time of scheduling my application service.
• If I pay my deposit, I will pay the remainder of my service upon completion of my application service.
• I will only use the proper products for my extensions.
• I will use the recommended brush to maintain my hair and keep from excess moisture by blow drying on a cool or warm setting, not hot: I will sleep with my hair in a loose, low ponytail or braid (Stylists will not be responsible for loose extensions because of moisture or improper brushing).
• I will rinse my hair after exposure to salt water.
• I will keep all my appointments in regards to the care of my extensions. (Stylists will not be responsible for dread-locked or damaged caused by missed appointments.
• I will not wear my extensions longer than recommended.
• I will not try to "fix" my own extensions, or cut them in any way.
• I will not try to remove my own extensions.
• I will follow all recommendations by my stylists, including when to replace extensions that can no longer be used.
• I will brush hair the recommended 2-3 times daily.
• I will notify my stylist that is stated above prior to the application appointment, if I change the color or texture of my natural hair between the consultation and the application appointment. A new consultation will be scheduled and a new deposit may be required.
• I will only have my stylist that is stated above and had my consultation with: apply my extensions, color and/or cut my hair after the extension application and while I am wearing my extensions, and I will also have my same stylist remove my extensions.

Must be signed by client 18 years or older. Clients under 18, must be signed by a parent or legal guardian.
