Food Concessionaire Vendor Form

We must have these completed and signed forms and a copy of your Insurance Certificate naming the Warren County Fair as additional insured NO LATER THAN MAY 1, 2025 to facilitate planning. We accept vendors on a FIRST COME Basis! You are responsible for the following: 1) Completed and signed Warren County Fair Application. 2) Completed and signed Warren County Health Department Application by June 1st. 3) Certificate of Insurance for $1,000,000 naming Warren County Fair as additional insured by June 1st. 4) Completed Electric Service Request (if electric is required). 5) Completed Storage Vehicle/Camping Form (if storage vehicle or camping is required) 6) List of foods you serve and pricing. 7) All new vendors-picture(s) of your concession must be submitted with application. 8) The County of Warren requires that all vendors be registered for a Vendor License, that application is attached to our application which can be downloaded from our website by June 1st. 8) PLEASE NOTE: No ATV, Golf Cart, or 4-Wheeler type vehicle are allowed on the grounds! NO ONE WILL BE ALLOWED TO SET-UP ON SITE FOR THE FAIR IF ALL PAPER WORK HAS NOT BEEN RECEIVED BY June 1, 2025.
