Home for Bark of an Angel Dog Rescue, I will need to carefully read the Foster Contract
and Foster Handbook in its entirety which is separate from this Application.
I understand this does not make me a
representative of the rescue. I am aware that the submission of this application does not
guarantee my approval to become a Foster Home. Once my application has been
approved, I agree to review the Foster Contract and Foster Handbook in its entirety prior
to being able to take my foster Dog/Puppy home. I have read this Application in its
entirety, and I agree that all statements contained in this document are made by me, and
are truthful to the best of my knowledge. I make this statement under the penalty of
perjury under the laws of the State of Illinois.
