Fiesta Days Entertainment Application

All entertainment slots for 2025 have been filled.

Applicants must submit this form to be considered for the entertainment at Spanish Fork Fiesta Days on July 24 at the City Park Stage. Priority will be given to performers who live or are from Spanish Fork. Performers donate their time (not paid). You will be notified by our chairmen if you are selected to perform. All entertainment must be family friendly.

Please bring your own beverages to keep hydrated while performing or waiting for your turn to perform. Vendors are available to purchase beverages if needed.

Liability Release/Waiver

The protection of Applicant's property and person is the sole absolute responsibility of Applicant. In Consideration for performance time and for other good and valuable consideration the Applicant hereby waives and releases the City and its agents, employees, representatives, elected and appointed officials etc. from any and all injuries, claims, damages, or other consequences suffered by Applicant as part of the Celebration.

VIDEO & PHOTOGRAPHY NOTICE: Spanish Fork City records all city events and broadcasts them on Spanish Fork 17, Spanish Fork YouTube, and city social media pages. Spanish Fork City also takes photographs at each event and uses them for promoting and advertising. By registering for events, you understand and consent to your image and sound being included in video, audio, or photography taken by Spanish Fork City and that such video, audio, or photography may be published on television, YouTube, and social media.

After you submit your application, you will receive a confirmation email with your application attached to it (please check your spam folder).

Your application isn't approved until you receive an email from our Committee Chair with further instructions. It may take a few months before a committee member contacts you regarding your application. If you have questions, please send an email to entertainment@spanishfork.gov
