All applicants must submit their completed application by March 31, 2020. Letters of Recommendation should be submitted by the individuals listed in the form below by March 31, 2020.

Applications will not be reviewed until all documents have been submitted to the admissions committee. Please note that once your application has been submitted, no additions or changes can be made.

You are required to submit two (2) letters of recommendation to the Admissions Committee. One letter must be from a physical therapist who supervised your observation hours. The second letter may be from anyone who may attest to your character outside of a family member. Please indicate below the individuals from whom we can expect to receive the recommendation form.
