This Training Contract is made and entered into by and between Michael Dow and Slash D Training, hereinafter designated "Trainer" and 

hereinafter designated "Owner." Trainer agrees to accept owner's dog,

for training; and it is the plan and the intention of the owner to contract said dog for training.

 1. Owner agrees that Michael Dow, Slash D Stockdogs, Slash D Quarter Horses, any facilities used for training and any facility owners are not liable for death, sickness, and/or accident including consequential damages caused to the dog while in training, except if caused by the willful and wanton negligence of the trainer. In addition, owner agrees to hold trainer completely harmless and not liable for any injury whatsoever caused to the owner, and/or any loss or damage to personal property.

2. Owner shall pay the trainer for boarding services; for BOARD, $100.00 per month, to include Black Gold dog food, cleaning, and bedding; and TRAINING, $600.00 per month, for the course of training set forth in this contract. A total of $700.00 per month, due at the time the dog is dropped off and every month thereafter that dog remains in training.

3. Training & Board fees are due upon arrival of your dog and due the 1st day of each month thereafter. A 10% late fee will be assessed for board/training fees not paid within 7 days of due date. Your spot cannot be guaranteed until your contract and first month’s payment have been received.

4. The dog shall be fully vaccinated (proof may be required) and be free of infectious, contagious, or transmissible disease. If your dog is found to be infectious, and you fail to inform trainer, and other kenneled dogs become infected with the same symptoms, the owner of the infectious dog will be held liable for any and all veterinarian bills that result from the transmission of the disease or parasite.

5. Trainer reserves the right to notify the owner within seven days of the dog's arrival if dog, in trainer's opinion, is deemed too unhealthy, unsound or too dangerous for trainer to handle or train. Owner is responsible for recovering dog immediately and for paying all fees incurred.

6. Trainer has the authority to contact a veterinarian for emergencies/regular care and owner will be liable for veterinarian's charges.

7. This contract is non-assignable and non-transferable. This contract represents the entire agreement between the parties. No other agreements or promises verbal or implied are included unless specifically stated. This contract is made and entered into in the State of Utah and will be interpreted under the laws of this state.
