This form MUST be submitted by April 20, 2026 for Miss and Teen titleholders, and Belles.

This photo should be at least 8x10, full color, and print quality (300 dpi) in .JPG, .JPEG, or .PNG format. Please label your photo "TitleHeadshot" (e.g. ColumbusHeadshot, or ColumbusTeenHeadshot, or ColumbusBelleHeadshot, or BelleHeadshot [use Belle's last only if you have not been assigned a Miss or Teen mentor]).

Head, hair, shoulders, arms, and sleeves must NOT be cropped! The Miss Georgia Organization will crop as needed.

DO NOT use "Miss" in this title.
